The News21 reporting project and the Center for Public Integrity conducted an analysis of aviation accident data which revealed that the major causes of aviation accidents included, ice buildup on aircraft, issues on runways, faulty aircraft repairs and maintenance and overtired pilots.

For over 40 years, the nation’s Transportation Safety Board has urged the FAA to initiate steps to reduce the probability of these types of accidents recurring.  The National Transportation Safety Board has issued more than 520 recommendations related to runways, fatigue, and icing.  Unfortunately, many of these recommendations have languished.  Many reasons exist as to why recommendations languish and some of these include, industries resist expensive fixes, unions resist work requirement changes, and it can take years of research and product development.

News21’s article stated that the FAA said in a prepared statement that “the agency agrees with the NTSB on a course of action most of the time.  When disagreements do arise, it’s usually over “whether voluntary action or rulemaking is the best approach or on the time required to accomplish a safety improvement.  We rarely disagree that some safety action is warranted.”
